Did you check with "sh pools" how many of your pool addresses are
allocated? We used to have a similar problem when we had "Auth Pool"
enabled, which (after an upgrade) lead to pool addresses not being
freed. Other than that, could you maybe post the radif output of 
one or two failed sessions (just say "radif" in the diag mode), it
would sure help debug the problem.
