(ASCEND) Strange Slot 4 Problem (still)

No one at all have any advise about Slot 4 always showing up as "not
available" on (2) Max 4004's?

Is it possible that TFTP of the setup for 1 bad unit, loaded into a good
unit, could result in such a problem?  I'm stumped, but don't believe
shipping could cause this same problem in 2 seperate units.
