You might also want to check if you are running on a 5ESS and if so, what 
generic your telco is running on it.  I just ran into this on Ohio with
Ameritech running a 5ESS.  They just reciently loaded a new generic onto
their switch.  A new provisioning field called "mc route index" now must
match your the route index for each PRI.  If it does not, you will get "all 
circuits are busy" if you call the number manually.  It took almost 30
days for Ameritech to figure this out.
