(ASCEND) ERROR_LMOD_SLOT_DOWN on a Max 2012 - 6.1.24

From the Ascend website:

	Warning 195 indicates the MAX unit is shutting down a 56K-modem slot
	card because too many calls are failing. Too many, in this case, are 8
	consecutive calls.  

Does this indicate that my modem card has gone bad or - what I suspect - that
one of our last modem users dials in 8 consecutive calls, fails to get a
connect (it being a modem and not isdn) and then the Max shuts down the modem
card?

After this, no modem calls are accepted at all since the card is shut down,
requiring a reset of the unit :-/
