This sounds mighty like you have implicitly denied everything in the
filters and have *not* included a rule to let ARP through. The result is
that initially things work OK but then stop working as the MAXes ARP cache
times out.

and specifically the last filter rule:
In filter 10...Valid=Yes
In filter 10...Generic...Forward=Yes
In filter 10...Generic...Offset=12
In filter 10...Generic...Length=4
In filter 10...Generic...Mask=ffff000000000000
In filter 10...Generic...Value=0806000000000000
Note: this is NOT an IP filter, it's a Generic filter.
Perhaps we could do with a FAQ entry like: "I put some IP filters on my
ethernet port; initially it was OK but a few minutes later it died"?
