I have seen this happen for two possible reasons before,

a> polarity (tip/ring) of the line out of the smart jack was backward
        that is for the cable between the jack and the P130
        you might need to build one that looks like this:
                1 -> 2
                2 -> 1
                4 -> 5
                5 -> 4

b> when using FDL on the line
        check to see if you have  Nailed T1->Mod Config->FDL=None

also, try setting Clock Source=No

does the router still die if it boots and then you connect the line?
what software rev?
