I've read (somewhere, can't remember where) that using ERROR is better 
than REJECT in /etc/mail/access.  Use lines like this:  

# domains you don't ever want to get email from -- REJECTS ENTIRE DOMAIN
@21cn.com	ERROR:"550 Too much spam"

# reject specific email address
Financial_Freedom_Awaits11 at yahoo.com    ERROR:"550 Too much spam"

sendmail has support for RBLs.  see cf/README in the sendmail source.

As far as rejecting on key words in headers, you're probably better off
implementing spamassassin or something similar.
