(ASCEND) Lan Security Error

We have running Livingston Radius with PM3 and USR Netserver-I for a 
few years without any problems! Now we have new Max 6000s installed 
working fine with majority of our customers!
 
However some of them can not get authenticated most of the time and I can 
not see a denied entry in the syslog coming from radius (where I normally 
do when they use an incorrect password, etc.) but we have Ascend 
reporting "LAN Security Error"! I could not find any references to this 
why it is happening! Below is from the syslog entry:

May 21 06:09:26 max1.mht.ts6.NHIX.NET ASCEND: slot 3 port 5, LAN security

Does anybody know why this might be happening?
