Yes to all...   Have had telco check translations....    We have 4 - 12 port
modem cards.  I see all...   I do take calls on 1 and second PRI.  Sometimes
(at least once a day now) ALL modems show in use and no one can connect
(busy).

First:
Does your 4004 see all it's modem cards?  Ours has 6 8port cards. If not,
reboot the max to get it to recognize all your cards.

Second:
Are you taking calls on the 2nd PRI?
Have you ever taken calls on the 2nd PRI?

If you answered 'NO' to either of these questions, do the following:

Unplug the first PRI, and see if calls come to the second PRI If this works,
plug the first back in.  Have the telco check the translations for your
trunkgroup, and verify that they have 46 B channels assigned to the group.
We've had the telco forget to increase the capacity to our trunkgroup on
many occasions, especially on FX lines.
