There are a couple of things to look at:

1) Make sure you're Radius accounting box isn't too heavily loaded.
2) Tweak the Accounting timeouts and Max retries on the TNT.

We were suffering from inaccurate Radius accounting logs at one point.

First, the Radius accounting info was written to a new dedicated 
accounting server (where the detail files are then sucked into database 
MySQL on a daily basis).

Next, we found that setting the Accounting Timeout to 30 seconds makes 
sure that we get all Stop records and no duplicates, either. You may also 
need to tweak your Accounting Max Retries too, although we run this on Zero.

I don't know the TNT commands, we're a Max 4000 shop.
