We actually found a place with the exact same fans except they are ball
bearing instead of sleave.  Exact same fan, exact same manufacturer.

It's the Elina Fan, Model KDB240615MB, purchased from BDS Electronics in
Santa Ana, CA.  The phone number I have for them is 714.662.1050
