(ASCEND) Motorola 56K Random Disconnects

Has anyone else had problems with the Motorla ModemSurfer 56K modem
randomly disconnecting after about 10 minutes?

I'm getting disconnect codes 11 and 45 which translate into:
 11  - DCD detected but went inactive
and
 45  - PPP - Receive   Terminate Req


Any reason I might be getting these?  What can I do to correct it?

I'm using a Max4000, firmware 5.0Ap16
