I think it's pretty clear, "Trust these authentication mechanisms, 
EXTERNAL, DIGEST-MD5 ...."

m4 only a preprocessor, postfix does this at run time (not via m4). The 
difference is that the sendmail way is much more powerful for those who 
have the know-how to directly edit their sendmail.cf
