I would think you could add a '\n' to hdr_format but I just tried it and 
it didn't work.  Might try it with a newer version of mutt, I know mine
is older.

from my .muttrc:

# index format is: NNN MM-DD HH:MM 
# 	where:	%3C  = number of message in NNN format
#		%D   = MM/DD/YY (see date(1))
#		HH = hour
#		MM = minute
#		%F = sender (left justified in field of 15)
#		%s = message subject

set hdr_format="%3C %Z %{%m-%d %H:%M} %-15.15F \n %s"
                                               ^
	adding newline char here didn't work --+
