I hear a lot of good things on the qmail list about Binc IMAP.  It supports Maildirs only I think.  It seems to support your minimum reqts.

I haven't used it, been using Courier as well for 2 years.  Kind of a pain to upgrade, but otherwise pretty happy with it.
