I use speakeasy and they use covad as the underlying telco. I looked at 
the covad ISP packages a while back and they seem quite similar to 
speakeasy. I have been using speakeasy for years hosting my domains.

I vote for covad and speakeasy :)
Qworst does not get my vote.
