Not sure about the levels part, but use the following line when you load
the module, so that joystick port gets properly initialized (this is a
line from my rc.modules file):

/sbin/modprobe es1371 joystick=0x200

If you have the driver built in to the kernel, use:

append="es1371=0x200"

in /etc/lilo.conf

Otherwise, you'll never be able to get a conventional joystick or pad
working ;-)
