I don't know if this will do all you need, but Dansguardian is working
great for us. You can customize your filters to just about any need, block
downloads, etc.. Logging can be varied to log everything, or only denied
attempts. We have ours set up so the access-denied page gives users the
ability to submit a form as to why they think a particular site should not
be blocked. It scans for weighted phrases, and denied words, rather then
using URLs. However you can add URLs if desired.
