If you're using PHP (and say, apache), and connecting to a LOCAL mysql
database, as in:

  mysql_connect("localhost", "user", "password")

you have to have something like the following in mysql:

  GRANT ALL ON *.* TO "user"@"localhost" IDENTIFIED BY "password";
